Yes, everything you need to put the exhaust on the model you select is included in the box along with a set of easy-to-follow instructions. Some of the bolts and brackets for the OEM exhaust may need to be re-used in order to facilitate the installation. So long as you have all of that, you should have no problem putting this, or any other Cobra exhaust on your machine.
On the 2 into 1 header how is the heat compared to dual exhaust

The difference between what you'll actually feel coming off the exhaust between a two-into-one and a dual exhaust setup is negligible. All of the thermal radiation comes from the motor, the exhaust just moves the gases along and out of the way. If anything, you'll feel a little more heat on both of your feet, calves and legs with a true dual exhaust as opposed to just the right side with a two-into-one pipe.
What is the actual pipe diameter of the head pipes? I have heard that it may be different sizes for different applications.

The Cobra PowerFlo RPT pipes to fit all 2010-2016 model year FLH models have a header pipe diameter of 1-7/8 in. that are covered by heat shields with a 2-1/2 in. diameter. They're all of the same diameter for those Harley Touring model applications.
